Community Health Needs Assessment/Community Health Improvement Plan
2023 Community Health Needs Assessment
Every 3 years, we conduct a Community Health Needs Assessment (CHNA) to ask residents about the most critical health needs in the McLennan County area. We listen, learn, and use the information we gather to improve our community’s overall health. The CHNA includes a CASPER (door-to-door survey), Community Survey (online and paper), Focus Groups, and Key Informant Interviews.
Top health issues that came out of the CHNA:
- CASPER: Mental Health, Diabetes Mellitus, Obesity, High Blood Pressure, and Aging Issues
- Community Survey: Mental Health, Diabetes Mellitus, Obesity, Aging Issues, and Women's Health
- Focus Groups: Mental Health, Diabetes Mellitus, Obesity, Heart Conditions, and Drug abuse
- Key Informant Interviews: Lack of Access to Care, Mental Health, and Chronic Health Conditions

2025 Community Health Improvement Plan
Building on insights from the most recent CHNA, the planning process for the Community Health Improvement Plan (CHIP) emphasized data-driven decision-making, community engagement, and alignment of local resources to address shared priorities. Through comparative analysis of existing CHNAs, CHIPs, and Improvement Plans from major community partners, the CHIP planning team identified key health issues that consistently affect McLennan County residents.
- Mental and Behavioral Health
- Healthcare Services
- Women's Health
- Non-Medical Drivers of Health (also called Social Determinants of Health)
